Ha! Jnorm. I got 100%. Woot. Guess that's why I'm Orthodox.
And I guess that's also why I need the Church. I'm apparently very prideful as seen in my "Ha!" response.

LOL!
It is the Son Who "sent" the Holy Spirit "from the Father" on the Apostles at Pentecost (John 15:26; Luke 24:49). All four answers were correct, since "sending", as opposed to "procession" or "spiration", is NOT a personal attribute. The MOST appropriate answer would've been #3: the Father AND the Son (John 14:26 AND John 15:26).
I got 90%. Something's wrong with your test. I answered #16 correctly but the test makers think that the Spirit comes only from the Father! :-)

I'm an RC, managed to get 80%. Some of the questions seemed to have multiple correct answers to me, the one Lvka mentions and the one about the end of salvation spring to mind... had to decide whether to give the answer I thought would be most likely to make me Orthodox or not.
